ABSTRACT

BACKGROUND/OBJECTIVES@#The diversity of meal provision places has increased in recent years and sodium intake can vary depending on where meals are eaten, particularly in large cities. In this study, an analysis of the recent trends in sodium intake was performed and a comparison of sodium intake level according to meal provision place among citizens of Seoul was performed. @*SUBJECTS/METHODS@#Data from a 24-h recall dietary intake survey from the 2010–2019 Korea National Health and Nutrition Examination Survey (KNHANES) were used in order to determine the trends in sodium intake among citizens of Seoul, aged 3–74 years old. (n = 11,811). The trend of daily sodium intake was presented in absolute amount and proportion compared to the chronic disease risk reduction intake (CDRR) for each selected characteristic. A comparison of sodium intake level according to meal provision place by sex and age groups as a total amount per meal (mg), density per meal (mg/1,000 kcal), and proportion of the daily sodium intake was performed using the 2016–2019 KNHANES. @*RESULTS@#Sodium intake levels showed a downward trend from 2010–2019. The highest level of sodium intake was observed for subjects aged 30–49, and the level for males aged 30–49 was 202.8% higher than the CDRR. Results of the analysis of sodium intake per meal according to meal provision place showed that the highest sodium intake was in the order of restaurant meal (RM) > institutional foodservice (IF) > home meal (HM) > convenience food (CF). A higher sodium density (mg/1,000 kcal) was observed for IF compared with RM in most adults. Adults aged over 50 years old consumed more than half of the daily sodium in HM. @*CONCLUSION@#Significant variation in the level of sodium intake was observed according to sex and age groups, therefore, different approaches and nutrition policies based on meal provision place are needed.

ABSTRACT

Laundry detergent pod (LDP) exposure has been reported to be fatal in children younger than 2 years, leading to respiratory or central nervous system depression. While gastrointestinal irritation is the most common symptom, there are reported cases of severe acidosis with respiratory depression or pneumonia, resulting in mortality. To our best knowledge, there is no report on a case of LDP exposure presenting with acute respiratory distress syndrome requiring extracorporeal membrane oxygenation support. Here, we present a case of a child with severe acute respiratory distress syndrome following LDP exposure, who was successfully treated with veno-pulmonary extracorporeal membrane oxygenation and steroids.

ABSTRACT

Purpose@#To evaluate the relationships between parameters of transcranial ultrasonography and results of visual field tests in patients with open angle glaucoma or suspected of having glaucoma. @*Methods@#This retrospective study was based on data from medical records of patients who visited the Department of Ophthalmology in Kangbuk Samsung Hospital from January 1, 2016, to October 17, 2019, and underwent transcranial Doppler ultrasonography as part of a routine health examination. Ophthalmic data were visual acuity, intraocular pressure, optical coherence tomography, and Humphrey visual field test results. Retinal nerve fiber layer defect was confirmed by a glaucoma specialist. Patients’ ophthalmic data, such as average ganglion cell layer thickness, visual field index, pattern standard deviation, and mean deviation, were divided into quartiles. Each ophthalmic artery parameter from transcranial Doppler ultrasonography was compared between quartiles. @*Results@#A total of 162 patients were reviewed. There was no difference in Doppler ophthalmic artery (OA) parameters between patients with or without retinal nerve fiber layer defect. None of the quartile groups of average ganglion cell layer thickness showed significant difference in any OA parameters. Patients in the low–visual field index quartile showed significant low peak systolic velocities of OAs when adjusted for age, sex, and presence of diabetes mellitus or hypertension (p = 0.016). A higher pattern standard deviation showed lower peak systolic velocity (p = 0.046). There was no significant tendency between any other OA parameter and mean deviation value. @*Conclusions@#Our study suggests that hemodynamic parameters of ophthalmic arteries might be associated with visual field status of patients. Further large-population studies are needed in order to better understand the relationship between visual function and ocular blood flow.

ABSTRACT

Purpose@#Exposure to heat and cold stress can negatively affect pregnant women. However, studies on the association between ambient temperature exposure and pregnant women, especially those with diabetes mellitus, are limited. The presence of this disease is likely to affect the woman’s capacity to adapt to the demands of pregnancy, consequently affecting fetal development. We examined the association between exposure to the mean temperature and diabetes mellitus during pregnancy. @*Methods@#Health Insurance Reviews and Assessment Service was used to obtain data on patients hospitalized between January 2015 and August 2022. The association between the number of pregnant patients with diabetes mellitus according to monthly mean temperature and seasonal differences was analyzed using a generalized additive model. The model was adjusted for the treatment date, mean humidity, and mean age of the women. @*Results@#The total number of patients with diabetes mellitus in pregnancy was 651,655 and the mean temperature was 12.87℃±9.89℃. The association between the mean temperature and diabetes mellitus during pregnancy showed a U-shaped nonlinear association. During the entire period, the number of patients with diabetes mellitus in pregnancy significantly increased by 2.82 per 1℃ increase in mean temperature (estimate, 2.82; 95% confidence interval [CI], -2.06 to 7.71). The number of patients significantly increased by 72.12 per 1℃ increase in mean temperature in the summer (estimate, 72.12; 95% CI, 50.36–93.89), but the increase was not statistically significant in the winter. @*Conclusion@#Our findings suggest that maternal exposure to mean temperature during pregnancy is associated with diabetes mellitus.

Purpose@#This study evaluated the validity of a physical activity classification table (PACT) based on total energy expenditure (TEE) and physical activity level (PAL) measured using the doubly labeled water (DLW) method in Korean adults and the elderly. @*Methods@#A total of 141 (male 70, female 71) adults and elderly were included. The reference standards TEEDLW , PALDLW were measured over a 14-day period using DLW. A 24-hour physical activity diary was kept for three days (two days during the week and one day on the weekend). PALPACT was calculated by classifying the activity type and intensity using the PACT. PALPACT was multiplied by resting energy expenditure measured by indirect calorimetry to estimate TEEPACT . @*Results@#The mean age of the study participants was 50.5 ± 18.8 years, and the mean body mass index was 23.4 ± 3.3 kg/m2 . A comparison of TEEDLW and TEEPACT by sex and age showed no significant differences. The bias, the difference between TEEDLW and TEEPACT , was male 17.3 kcal/day and female −4.5 kcal/day. The percentage of accurate predictions (values within ± 10% of the TEEDLW ) of TEEPACT was 58.6% in males and 54.9% in females, with the highest prediction values in the age group 40–64 years (70.9%) in males and over 65 years (73.9%) in females. The spearman correlation coefficient (r) between TEEPACT and TEEDLW was 0.769, indicating a significant positive correlation (p < 0.001). @*Conclusion@#In this study, the use of a new PACT for calculating TEE and PAL was evaluated as valid. A web version of the software program and a smartphone application need to be developed using PACT to make it easier to apply for research purposes.

ABSTRACT

A 58-year-old woman visited the hospital complaining of fatigue and indigestion lasting for more than 3 months. She had no medical history other than taking a calcium plus vitamin D supplement for osteopenia. The initial blood test showed a high calcium level of 14.0 mg/dL. Additional tests were performed to differentially diagnose hypercalcemia. The blood test results were as follows: serum parathyroid hormone (PTH)=247.0 pg/mL, PTH-related peptide <1.0 pg/mL, phosphorous=2.6 mg/dL, 25-hydroxy-vitamin D=14.5 pg/mL, creatinine=1.09 mg/dL, and 24 hr urine calcium=215 mg/dL. A 4.5 cm sized cystic lesion on the intra-thyroidal space was confirmed on neck sonography and 4-dimensional parathyroid computed tomography, but technetium-99m methoxyisobutylisonitrile parathyroid scintigraphy showed equivocal results. After removal of the cystic lesion, serum calcium and PTH were normalized, and parathyroid lipoadenoma was confirmed in the postoperative pathology. Clinical features of parathyroid lipoadenoma are known to be similar to common parathyroid adenoma, but imaging studies often report negative findings. Therefore, it is necessary to better understand this rare disease for the differential diagnosis. For the final diagnosis and treatment of this disease, parathyroidectomy with intraoperative PTH measurement may be required.

ABSTRACT

Objective@#Isotretinoin should not be used during pregnancy because of the risk of birth defects. Most pregnant women exposed to isotretinoin choose voluntary pregnancy termination due to concerns about birth defects. However, birth outcome data supporting the termination of pregnancy are lacking. This study aimed to evaluate pregnancy and neonatal outcomes after periconception exposure to isotretinoin. @*Methods@#This was a prospective cohort study. We evaluated pregnancy and neonatal outcomes after exposure to isotretinoin in 151 pregnant women. Among 1,026 callers at the Korean Teratology Information Service from 2001 to 2017 exposed to isotretinoin during the periconception period, 151 pregnant women who received counseling on teratogenic risk after visiting the clinic were included. @*Results@#Among the 151 participants who visited the clinic, only 42 were evaluated using ultrasonography until approximately 20 weeks of gestation. Ultimately, 23 patients were included in the study. The average gestation period during the last exposure to the drug was 2 weeks, and the average daily exposure dose was 12 mg. There were two cases of major birth defects in the exposure group. Spontaneous abortion rates were 17.7% and 8.7% in the exposure and nonexposure groups, respectively (P=0.035). There was no significant difference between the exposure and non-exposure groups in terms of pregnancy and neonatal outcomes. @*Conclusion@#There was no significant difference in pregnancy and neonatal outcomes, including birth defects, between the exposure and non-exposure groups. Further studies with larger sample sizes are required to validate our findings.

ABSTRACT

Purpose@#We report a case of trochlear nerve palsy caused by a superior cerebellar artery (SCA) aneurysm.Case summary: A 34-year-old woman visited our clinic complaining of distance diplopia and a temporal headache 7 days in duration. She had no previous relevant medical or trauma history. Her visual acuity; intraocular pressure; and pupil, anterior segment, and fundus evaluations were unremarkable. Extraocular muscle examination (EOM) revealed six prism diopters (PD) of right hypertropia in the primary gaze; this worsened when the head was tilted to the right. Brain magnetic resonance imaging revealed a round solid nodule between the temporal lobe and pons; we thus suspected a petrous ridge meningioma. She was referred to our neurosurgery department for meningioma treatment. However, 1 hour later, she visited our emergency room with severe headache and nausea. Brain computed tomography angiography revealed a subarachnoid hemorrhage and a ruptured SCA aneurysm. Emergency coil embolization was successfully performed. Four weeks after surgery, the right hypertropia was slightly decreased. Four months later, the diplopia had disappeared and EOM revealed orthotropia. @*Conclusions@#SCA aneurysms are rare and can be misdiagnosed even after radiological examination. Our case emphasizes that an SCA aneurysm should be considered during the differential diagnosis of patients with diplopia and headache.

ABSTRACT

Objectives@#Given that cardiopulmonary resuscitation (CPR) is an aerosol-generating procedure, it is necessary to use a mechanical ventilator and reduce the number of providers involved in resuscitation for in-hospital cardiac arrest in coronavirus disease (COVID-19) patients or suspected COVID-19 patients. However, no study assessed the effect of changes in inspiratory time on flowrate and airway pressure during CPR. We herein aimed to determine changes in these parameters during CPR and identify appropriate ventilator management for adults during CPR. @*Methods@#We measured changes in tidal volume (Vt), peak inspiratory flow rate (PIFR), peak airway pressure (Ppeak), mean airway pressure (Pmean) according to changes in inspiratory time (0.75 s, 1.0 s and 1.5 s) with or without CPR. Vt of 500 mL was supplied (flowrate: 10 times/min) using a mechanical ventilator. Chest compressions were maintained at constant compression depth (53 ± 2 mm) and speed (102 ± 2/min) using a mechanical chest compression device. @*Results@#Median levels of respiratory physiological parameters during CPR were significantly different according to the inspiratory time (0.75 s vs. 1.5 s): PIFR (80.8 [73.3 – 87.325] vs. 70.5 [67 – 72.4] L/min, P < 0.001), Ppeak (54 [48 – 59] vs. 47 [45 – 49] cmH2O, P < 0.001), and Pmean (3.9 [3.6 – 4.1] vs. 5.7 [5.6 – 5.8] cmH2O, P < 0.001). @*Conclusions@#Changes in PIFR, Ppeak, and Pmean were associated with inspiratory time. PIFR and Ppeak values tended to decrease with increase in inspiratory time, while Pmean showed a contrasting trend. Increased inspiratory time in low-compliance cardiac arrest patients will help in reducing lung injury during adult CPR.

Background and Objectives@#Despite recent improvements in advanced life support, the overall survival rate after cardiac arrest remains low. We aimed to examine the association of a multidisciplinary team approach with clinical outcomes in patients undergoing extracorporeal cardiopulmonary resuscitation (ECPR) in the emergency department (ED). @*Methods@#This retrospective, single-center, observational study included 125 patients who underwent ECPR in the ED between May 2004–December 2018. In January 2014, our institution implemented a multidisciplinary extracorporeal membrane oxygenation (ECMO) team. Eligible patients were classified into pre-ECMO-team (n=65) and post-ECMO-team (n=60) groups. The primary outcome was in-hospital mortality. @*Results@#In-hospital mortality (72.3% vs. 58.3%, p=0.102) and poor neurological outcomes (78.5% vs. 68.3%, p=0.283) did not differ significantly between the pre- and post-ECMOteam groups. However, among the 60 patients who experienced in-hospital cardiac arrest,in-hospital mortality (75.8% vs. 40.7%, p=0.006) and poor neurological outcomes (78.8% vs. 48.1%, p=0.015) significantly decreased after the multidisciplinary team formation.Multivariable logistic regression analysis showed that the multidisciplinary team approach (adjusted odds ratio, 0.20; 95% confidence interval, 0.07–0.61; p=0.005) was an independent prognostic factor for in-hospital mortality in in-hospital cardiac arrest patients. @*Conclusions@#A multidisciplinary team approach was associated with improved clinical outcomes in in-hospital cardiac arrest patients undergoing ECPR in the ED. These findings may help in improving the selection criteria for ECPR in the ED. Further studies to overcome the study limitations may help improving the outcomes of out-of-hospital cardiac arrest patients.

This systematic review and meta-analysis aimed to evaluate the rates of major malformation (MM) among isotretinoin-exposed pregnant women over the years through a systematic review and meta-analysis. @*Methods@#Eligible studies were searched and identified using various databases. Single-arm meta-analysis and meta-analysis of odd ratios among controlled studies were performed using Review Manager version 5.3. @*Results@#Ten eligible studies that combined 2,783 isotretinoin-exposed women were included in our study. The rate of MM weighted for the sample size was 15%. Three studies that included an unexposed comparison group were eligible for the meta-analysis. The pooled odds ratio of MM for isotretinoin-exposed women was 3.76. After 2006, the pooled odds ratio of MM for isotretinoin exposure was significantly lower at 1.04. @*Conclusion@#The current rate of MM in isotretinoin-exposed women was substantially lower after 2006.

Background@#We performed this study to establish a prediction model for 1-year neurological outcomes in out-of-hospital cardiac arrest (OHCA) patients who achieved return of spontaneous circulation (ROSC) immediately after ROSC using machine learning methods. @*Methods@#We performed a retrospective analysis of an OHCA survivor registry. Patients aged ≥ 18 years were included. Study participants who had registered between March 31, 2013 and December 31, 2018 were divided into a develop dataset (80% of total) and an internal validation dataset (20% of total), and those who had registered between January 1, 2019 and December 31, 2019 were assigned to an external validation dataset. Four machine learning methods, including random forest, support vector machine, ElasticNet and extreme gradient boost, were implemented to establish prediction models with the develop dataset, and the ensemble technique was used to build the final prediction model. The prediction performance of the model in the internal validation and the external validation dataset was described with accuracy, area under the receiver-operating characteristic curve, area under the precision-recall curve, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Futhermore, we established multivariable logistic regression models with the develop set and compared prediction performance with the ensemble models. The primary outcome was an unfavorable 1-year neurological outcome. @*Results@#A total of 1,207 patients were included in the study. Among them, 631, 139, and 153were assigned to the develop, the internal validation and the external validation datasets, respectively. Prediction performance metrics for the ensemble prediction model in the internal validation dataset were as follows: accuracy, 0.9620 (95% confidence interval [CI],0.9352–0.9889); area under receiver-operator characteristics curve, 0.9800 (95% CI, 0.9612– 0.9988); area under precision-recall curve, 0.9950 (95% CI, 0.9860–1.0000); sensitivity, 0.9594 (95% CI, 0.9245–0.9943); specificity, 0.9714 (95% CI, 0.9162–1.0000); PPV, 0.9916 (95% CI, 0.9752–1.0000); NPV, 0.8718 (95% CI, 0.7669–0.9767). Prediction performance metrics for the model in the external validation dataset were as follows: accuracy, 0.8509 (95% CI, 0.7825–0.9192); area under receiver-operator characteristics curve, 0.9301 (95% CI, 0.8845–0.9756); area under precision-recall curve, 0.9476 (95% CI, 0.9087–0.9867); sensitivity, 0.9595 (95% CI, 0.9145–1.0000); specificity, 0.6500 (95% CI, 0.5022–0.7978); PPV, 0.8353 (95% CI, 0.7564–0.9142); NPV, 0.8966 (95% CI, 0.7857–1.0000). All the prediction metrics were higher in the ensemble models, except NPVs in both the internal and the external validation datasets. @*Conclusion@#We established an ensemble prediction model for prediction of unfavorable 1-year neurological outcomes in OHCA survivors using four machine learning methods. The prediction performance of the ensemble model was higher than the multivariable logistic regression model, while its performance was slightly decreased in the external validation dataset.

ABSTRACT

Objective:To analyze the clinical effect of traditional Chinese medicine Lianhua Qingwen in the treatment of coronavirus disease 2019 (COVID-19) and provide the basis for medication guides through a retrospective study in a cohort of COVID-19 confirmed patients. Method:A retrospective analysis of clinical records was conducted in COVID-19 confirmed patients at The Ninth Hospital of Wuhan and CR&WISCO General Hospital including the treatment group (21 patients, basic treatment in combination with Lianhua Qingwen granules, 1 packet/time, 3 times/day) and the control group (21 patients, basic treatment). Comparison between the two groups was made in terms of the disappearance rates of cardinal symptoms (fever, cough and weakness), duration of fever, and disappearance rates of other symptoms (muscle pain, expectoration, nasal obstruction, running nose, dry throat, pharyngalgia, shortness of breath, chest distress, dyspnea, dizziness, headache, nausea, vomiting, loss of appetite and diarrhea). Result:The baseline data were similar between the two groups. When compared with the control group, patients in the treatment group had the higher clinical effect, including the disappearance rate of fever (85.7% vs 57.1%, χ2=4.200, P=0.040), the disappearance rate of cough (46.7% vs 5.6%, P=0.012), the disappearance rate expectoration (64.3% vs 9.1%, P=0.012), the disappearance rate of shortness of breath (77.8% vs 0, P=0.021), and the duration of fever [(4.6±3.2) d vs (6.1±3.1) d, P=0.218]. Conclusion:Lianhua Qingwen can significantly relieve cardinal symptoms in COVID-19 confirmed patients by inhibiting fever and cough, reducing their duration, as well as improving individual symptoms. All these results provide preliminary clinical evidence for Lianhua Qingwen granules in the COVID-19 treatment.

ABSTRACT

Background@#Suicide among the elderly is a public health concern, as life expectancy is increasing rapidly and suicide rates increase with age. In Korea, self-poisoning is the most common method of attempted suicide. This study aimed to evaluate the characteristics of attempted suicide by self-poisoning among the elderly and to identify risk factors related to the suicide attempts. @*Methods@#A cross-sectional observational study was conducted using the Emergency Department-based Injury In-depth Surveillance database in Korea. We included all adult patients visiting the emergency department (ED) who attempted suicide by poisoning between January 2011 and December 2017 and stratified according to age: the elderly (≥ 65 years old) and the younger group. Characteristics and risk factors for attempted suicide by poisoning among the elderly were evaluated using stepwise regression analysis. @*Results@#Among 25,904 adult patients, 5,164 (19.9%) were classified as elderly. The elderly were more likely to be admitted to hospital and intensive care units, the average ED length of stay was longer, and total mortality was higher than that of the younger group. Male sex, low socioeconomic status, poor physical health, pesticide use, lower alcohol consumption, and fewer prior suicide attempts were found to be risk factors for suicide among the elderly. @*Conclusion@#Self-poisoning among the elderly is associated with poorer clinical outcomes than in younger adult patients. Suicide among the elderly is a potentially preventable public health problem, and with proper identification of the associated risk factors, appropriate multidisciplinary intervention strategies can be implemented.

Background@#Coffee is one of the most commonly consumed beverages in the world. There is evidence that the consumption of coffee has a strong influence on health outcomes. However, the relationship between coffee consumption and serum uric acid in the Korean population is unclear. In this study, we investigated the relationship between coffee consumption and serum uric acid levels in Korean adults. @*Methods@#This study included 2,966 adults aged ≥19 years who participated in the Korea National Health and Nutrition Examination Survey 2016. The participants were divided into four groups according to the amount of coffee consumed and serum uric acid level. Linear regression analysis was used to analyze the relationship between coffee consumption and serum uric acid level. @*Results@#Serum uric acid level increased with increasing coffee consumption (P<0.001). After adjusting for all confounding factors, serum uric acid level was higher in the groups that consumed coffee daily, at more than four teaspoons, than in the groups that did not consume coffee (P<0.001). @*Conclusion@#The findings of this study suggest that coffee consumption has a positive relationship with serum uric acid level.

Objective@#This study examined whether the preventive effects of the safety belt on traumatic brain injury (TBI) from motor vehicle collisions (MVCs) differ according to the occupants’ age. @*Methods@#This study was a retrospective, observational study. This study evaluated the crash data from 2011 to 2016 obtained from the Emergency Department-based Injury In-depth Surveillance registry. The injured occupants were categorized by age into young adults (age, 18-35 years; n=35,032), middle-aged adults (age, 36-55 years; n=34,507), and older adults (aged older than 55 years, n=21,895). The primary (TBI), secondary (intensive care unit [ICU] admission), and tertiary endpoint (mortality) were set. Multivariate logistic regression analysis was performed, and the adjusted odds ratios (aORs) of subgroups were calculated for study outcomes adjusted for any potential confounders. @*Results@#Among 91,434 patients, 61,205 used seat belts at the time of the crashes. Compared to the unbelted group, the belted group was less likely to have a TBI. A comparison of the aOR of subgroups for TBI revealed the odds ratio reduction to be the highest in young adults (aOR, 0.39; 95% confidence interval [CI], 0.32-0.47), followed by middle-aged adults (aOR, 0.39; 95% CI, 0.33-0.47) and older adults (aOR, 0.49; 95% CI, 0.42-0.56). In addition, seat belt use had a preventive effect on ICU admission and mortality at all subgroups, the effect of which decreased with age. @*Conclusion@#The protective effects of seat belts on TBI, ICU admission, and mortality from MVCs were reduced with age.

ABSTRACT

OBJECTIVE: This study examined the clinical effects of discharge against medical advice (DAMA) on suicide reattempts and clarified the risk factors related to DAMA. METHODS: A cross-sectional observational study was conducted in the emergency department (ED) of Chungbuk National University Hospital from 2015 to 2017. The medical records of suicide attempters from the National Emergency Department Information System were reviewed. The primary outcome was the ED revisit rate. The characteristics of the patients associated with DAMA were also examined. RESULTS: Among 889 eligible suicide attempters, 328 patients (36.9%) were reported to have DAMA at least once during the study period. Patients who were discharged against medical advice were more likely to reattempt suicide compared to normal disposition patients (11.0% vs. 3.7%, adjusted odds ratio [AOR], 3.002; 95% confidence interval [CI], 1.71–5.28). The independent risk factors for DAMA were age≤60 (AOR, 1.77; 95% CI, 1.20–2.59), female (AOR, 1.45; 95% CI, 1.09–1.91), ED visit at night time (AOR, 1.41; 95% CI, 1.03–1.92), and ED discharge at night time (AOR, 1.40; 95% CI, 1.06–1.85). CONCLUSION: Patients who were discharged against medical advice revisited the ED more after suicide attempts. Public efforts will be needed for patients who are discharged against medical advice considering those risk factors.
